Sponsor Assessment Form (IMM 5492)

This video will show you how to fill Sponsor Assessment (IMM 5492)

This form must be completed by:

  1. Cosponsor
  2. CG representative, usually the priest.



Settlement Plan (IMM 5440)

In this video, we will show how to fill the Settlement Plan for Sponsorship Agreement Holders. (IMM 5440).

Please pay attention to the following notes:

  1. All cosponsors should be as precise and detailed as possible. Do not leave any questions blank.
  2. The settlement plan must address the unique needs of the refugees in each sponsorship application.
  3. Your answers should not be copied from other settlement plans.
  4. The Settlement Plan must include the settlement needs of every refugee applicant named on the Sponsorship Undertaking, even if they will not come to Canada with the principal applicant.



Generic Application Form (IMM 0008)

In this video, we will show how to fill in the Generic Application Form (IMM 0008) for Canada. Which is one of the forms used for the private sponsorship program.




Schedule A: Background/Declaration Form (IMM 5669)

In this video, we will show how to fill Schedule A – Background Declaration. (IMM 5669).

This form must be completed by:

The principal applicants, the spouse or common-law partner, (whether accompanying them to Canada or not); and their dependent children aged 18 years or older (whether accompanying them to Canada or not).




Schedule 2: Refugees Outside Canada Form (IMM0008/Schedule 2)

In this video, we will show how to fill Schedule 2, Refugees Outside Canada (IMM 0008 Schedule 2)

This form must be completed by:

The principal applicants, the spouse or common-law partner, (whether accompanying them to Canada or not); and their dependent children aged 18 years or older (whether accompanying them to Canada or not).




Appointment of Representative

In this video, we will show how to fill the Appointment of Representative(s) in the Expected Community of Settlement (IMM 5956).

This form has to be filled by cosponsors and/or Constituent Groups representatives if the newcomers whom they have sponsored, choose to reside out of their settlement area.

For example, a refugee who was originally destined to Toronto decides to move to Windsor. 

The office for refugees archdiocese of Toronto Office works only within the Greater Toronto Area, which means that after their arrival to Canada, newcomers are expected to reside within the GTA area. However, as permanent residents, newcomers can move out and choose to settle in a different area.




How to download Immigration forms

If the forms do not open on your internet browser, please do the following:

1.Right-click on the form;

2.Click “Save As” and save the document to your desktop/laptop; and

3.Go to the saved file and open it using Adobe Acrobat or any program that opens PDF files
